This supplement to Federal Practitioner, the second in a two-part series, contains the appendices to the official VHA/DoD clinical practice guideline and pre-end-stage kidney disease. It also includes the complete bibliography for the guideline and an addendum containing an article (reprinted from Blood Purification, volume 20, number 1, with permission from the publisher) about the National Kidney Foundation's Kidney Disease Outcomes Quality Initiative (K/DOQI) guidelines, which are in various stages of development and publication. The first part of this Federal Practitioner supplement series, published in August, contained the VHA/DoD guideline algorithm and text. The current VHA/DoD guideline was developed by a panel that included internists, family practitioners, nephrologists, nurses, pharmacists, and policy makers from the VHA, academia, and a team of private guideline facilitators. The development process was evidence-based whenever possible. Where evidence was ambiguous or conflicting, or where scientific data were lacking, the clinical experience of the expert panel guided the development of consensus-based recommendations.
